A man pleaded guilty to felony premium fraud and was sentenced to 180 days in jail, two years of formal probation, and order to pay $225,168 in restitution. The man owned a company providing security guard, staffing, and patrol services. The Department received a fraud referral stating that the man had failed to report an employee injury, and that the man had seriously underreported employee payroll. An audit by the Department revealed $3,431,903 in undisclosed payroll from 2017 through 2023, which resulted in unpaid workers compensation premiums of an estimated $205,565
