The Delaware Department of Insurance published a Bulletin informing all insurers, brokers, and agents writing auto insurance in the state on the requirement of the use of certain forms. Effective July 14, 2024, the use of these forms will be mandated, and no other format for the required information will be accepted. The three forms are available on the Delaware DMV website.

FR1–Delaware Uninsured Vehicle Report is the form insurers must use to report any person involved in an auto accident who does not have auto insurance to the DMV. The insurer should report the person's name, address, and description of the vehicle. 

FR4–Notice of Cancellation of Termination of Policy is the form insurers must use to notify the DMV of the cancellation or termination of any private passenger automobile insurance policy. They should include reason for termination and the policyholder's name, address, and description of the vehicle.

FR19–State of Delaware Insurance Certification is a form where the insurer certifies it issued a liability insurance policy for the vehicle(s) listed, and that they meet the minimum limits required by Delaware Iaw. The form should also certify that the policy is in force on the certification date and in effect during the state policy period.
