A woman has been charged with insurance fraud and forgery for her claims of injury after an auto accident. She filed a claim for injury after an accident and provided an invoice for treatment from a local hospital. However, the hospital did not have any record of the woman receiving any treatment from them for the date in question, and it had no invoices that matched what she had submitted to her insurer. Later that year she submitted another claim for injury after an accident and again submitted another forged invoice.
