A man hired a rideshare vehicle to take him to the mall and claimed that the driver ran over his foot as he was exiting the vehicle. He submitted medical bills for his injuries to the company for reimbursement. When the company contacted the medical providers to verify the claim, the medical provider advised that the man had never received treatment from the facility and that the documents were forged. Warrants have been issued against the man for insurance fraud and forgery in the first degree.
