A federal regulator said Monday that lapses in Black Elk Energy Offshore Operations LLC's supervision of its contractors degraded the safety culture at an offshore platform in the Gulf of Mexico, where a November 2012 explosion killed three Filipino workers.
The Bureau of Safety and Environmental Enforcement, which polices offshore drilling, issued a report that also cited Black Elk's contractors for inadequately training workers and failing to follow Black Elk's safety guidelines.
